ABSTRACT
Since the relation between the incidence of cardiovascular diseases and the use of antiepileptics are brought forward, in this study it is aimed to search and discuss whether the carbamazepine results some changes or not on the serum lipid and lipoproteins. The study included 30 patients diagnosed as epilepsy and had carbamazepine for at least 1 years for this reason. Out of these patients 20 were male and 10 were female. Thirty healthy persons consisting of 14 females and 16 males and not having any certain disease were chosen as the control group. To determine the level of the serum lipid of the patients and the control group, blood samples were taken after a night spent without having anything to eat. The HDL-cholesterol, total cholesterol and triglycerid levels were measured by using routine laboratory processes. The LDL-cholesterol level and the LDLI HDL-cholesterol ratios were calculated. The results were analysed statistically by using the student's t-test. As a result of the study it was found that there was a statistically significant increase in the level of the HDL-cholesterol and triglycerid of the patients taking carbamazepine relative to the controls. It was determined that there was a decrease in the level of serum total cholesterol of the patients group relative to the control group and this decrease was not significant statistically. But the LDL-cholesterol level decreased in the patients taking carbamazepine. When the LDLI HDL-cholesterol ratio was calculated, it was seen that the level was lower in the patients taking carbamazepine than those in the control group. These two results were found statistically quite significant. All these results, the increase in the HDL-cholesterol, the decrease in the LDL-cholesterol and, the more important the decrease in the LDLIHDL-cholesterol ratio let us to think about the fact that the long-term usage of the carbamazepine decreases the incidence of the atherosclerosis and coroner heart disease.
Kardiovasküler hastalıkların insidansı ve antiepileptiklerin kullanımı arasında ilişki olduğu ileri sürüldüğünden, bu çalışmada karbamazepinin serum lipid ve lipoproteinleri üzerinde değişiklik yapıp yapmadığının araştırılıp tartışılması amaçlanmıştır. Çalışmaya epilepsi tanısı alan ve bu nedenle en az bir yıldır karbamazepin kullanan 10'u kadın, 20'si erkek 30 hasta katıldı. Kontrol grubu bilinen belirli bir hastalığı olmayan 14 kadın, 16 erkekten oluşan 30 sağlıklı kişi olarak seçildi. Hastaların ve kontrol grubunun serum lipid düzeyi tayini aç bir geceden sonra alınan kan örneklerinde yapıldı. HDL-kolesterol, total kolesterol ve trigliserid düzeyleri rutin laboratuvar işlemleriyle ölçüldü. LDL-kolesterol düzeyi ve LDL/HDL-kolesterol oranı saptandı. Sonuçlar student's t-testi kullanılarak istatistiksel olarak analiz edildi. Çalışmamız sonucunda karbamazepin kullanan hastaların HDL-kolesterol ve trigliserid düzeyinde kontrol grubuna göre istatistiksel olarak anlamlı bir artış olduğu görüldü. Serum total kolesterol düzeyinde kontrol grubuna göre hasta grubunda bir düşüş olduğu ancak bunun istatistiksel açıdan anlamlı olmadığı saptandı. LDL-kolesterol düzeyi ve LDL/HDL-kolesterol oranı karbamazepin kullananlarda kontrol grubuna göre düşük bulundu. Bu iki sonuç istatistiksel olarak ileri derece anlamlı idi. HDL-kolesterolünde artış, LDL-kolesterolünde düşme ve daha da önemli olarak LDL/HDL-kolesterol oranında düşme saptanması, uzun süreli karbamazepin kullanımının ateroskleroz ve koroner kalp hastalığı insidansını azalttığını düşündürmektedir.
