ABSTRACT
A thirty-six-year-old woman experienced tonoclonic seizures seven times in the last 24 hours. On neurological examination, she could neither speak nor comply with instructions. EEG showed bilateral spike-waves. She had another tonic-clonic seizure and treated with intravenous diphenylhydantoin. Eighteen hours after the control of tonic-clonic seizures, she began to speak and comply with instructions; however, confusion continued during the following 72 hours and EEG abnormality persisted over the following eight days without abnormal neurological signs. A diagnosis of nonconvulsive status epilepticus following convulsive status epilepticus was made.
Otuz altı yaşındaki kadın hasta son 24 saatte yedi kez tonik-klonik nöbet geçirme yakınmasıyla başvurdu. Muayene sırasında hasta konuşmuyor ve muayene komutlarını yerine getirmiyordu. EEG'sinde bilateral yaygın diken dalgalar görüldü. Bu sırada tekrar tonik-klonik nöbet geçiren ve nöbetleri intravenöz difenilhidantoin ile kontrol altına alınan hasta, son nöbetten 18 saat sonra konuşmaya ve komutları yerine getirmeye başladı fakat konfüzyonu 72 saat, EEG anormalliği klinik bulgular olmadan sekiz gün boyunca devam etti. Bu bulgularla hastaya konvulsif status epileptikusu izleyen nonkonvulsif status epileptikus tanısı kondu.
