ABSTRACT

We investigated the effects of hepatic enzyme-inducing antiepileptic drugs on serum sex hormone levels in male epileptic patients. Patients and Methods: The levels of free testosterone and sex hormone-binding globulin (SHBG) were measured in 51 epileptic male patients (mean age 32.5 years; range 17-55 years) receiving carbamazepine monotherapy (n=28), phenytoin monotherapy (n=12), and polytherapy (n=11). The results were compared with those of 34 healthy age-matched controls (mean age 32.4 years; range 19-51 years). Results: No significant differences were found in the mean free testosterone levels between the four groups. However, all antiepileptic drug-treated patient groups had significantly higher SHBG levels than the control group (p=0.001). The mean SHBG levels in the patient groups did not show any significant relationship with the duration of epilepsy, duration of drug treatment, and the frequency and type of seizures. Conclusion : Our findings corroborate the concept that carbamazepine and phenytoin directly promote SHBG synthesis by enzyme induction.

Antiepileptik Tedavi Gören Erkek Hastalarda Serbest Testosteron ve Seks Hormonu Bağlayan Globulin Düzeyleri

Epilepsili erkek hastalarda karaciğer enzimlerini indükleyici antiepileptik ilaçların plazma seks hormon düzeyleri üzerindeki etkileri araştırıldı. Hastalar ve Yöntemler: Karbamazepin monoterapisi (n=28), fenitoin monoterapisi (n=12) ve politerapi (n=11) gören 51 erkek hastada (ort. yaş 32.5; dağılım 17-55) serbest testosteron ve seks hormonu bağlayan globulin (SHBG) düzeyleri öl- çülerek bulgular, kontrol grubundaki 34 sağlıklı ki- şinin (ort. yaş 32.4; dağılım 19-51) sonuçlarıyla karşılaştırıldı. Bulgular: Ortalama serbest testosteron düzeyleri açısından gruplar arasında anlamlı bir farklılık saptanmadı. Ancak, ilaç kullanan bütün hasta gruplarında SHBG düzeyleri, kontrol grubundan anlamlı derecede yüksek bulundu (p=0.001). Hasta grupları arasında ortalama SHBG düzeyleri ile hastalık süresi, ilaç kullanma süresi, nöbet sıklığı ve nöbet tipleri açısından anlamlı bir ilişki gözlenmedi. Sonuç: Bulgularımız, karbamazepin ve fenitoinin enzim indüksiyonu yoluyla SHBG sentezini doğrudan artırdığı yönündeki görüşleri desteklemektedir.
